Notice

Notice is hereby given that:

  1. the information requested and provided by you in this application form is for the purposes of and in relation to your application to be a distributor for the Company. You are at liberty to decide whether to provide such information as requested but should you decide not to provide such information as completely, the Company has the discretion whether to approve your application due to insufficiency of information.
  2. should your application is approved, in addition to these information as provided, the Company may request further related information and will keep records of information in respect of your points value, calculation of income derived from your activities transacted for the business purposes for which you are a distributor of the Company and any related information thereof.
  3. as long as you are a distributor of the Company and for the purposes of the Company’s business operation including compliance with the statutory obligations, the aforesaid information as may be provided by you or as may be derived, maintained and kept by the Company in the course of your activities as a Distributor of the Company shall be maintained, shared, used, disclosed, distributed and circulated to the related individuals in accordance with the Company’s business and marketing plan and any regulatory bodies to which the Company’s business relates including but not limited to your distribution up-line/leaders and our ultimate holding company in New Zealand.
  4. you are at liberty not to provide any such information as requested in the course of you being a Distributor of the Company or to limit the processing of such information by giving written notification to the Company.  However should such refusal or limitation in the provision of such information affect the proper conduct of the Company’s business and marketing plan in relation to you, the Company shall have the right to terminate you as a Distributor of the Company.
  5. you have the right to request, access to and to request correction of any information as may be maintained and kept by the Company in relation to your activities as a Distributor of the Company including but not limited to the aforesaid information.

Declaration

I hereby declare that I have read and understood the contents of the above Notice and hereby give my consent to the Company to collect, process, maintain, use and disclose such information as may be provided by me in this application and should I be approved to be a Distributor of the Company, such other information for the purposes as stated in the above Notice.
